The effect of time interval on esophagectomy after neoadjuvant treatment.

چکیده
"Esophagectomy timing after neoadjuvant therapy for distal esophageal adenocarcinoma" published in the Annals of Thoracic Surgery (doi: 10.1016/j.athoracsur.2015.09.044) states that 30- and 90-day mortality after esophagectomy for adenocarcinoma in patients with neoadjuvant chemoradiation, is significantly higher in patients with a postradiation interval of 9 weeks or more. The authors suggest that a "wait and see" approach after neoadjuvant therapy for esophageal adenocarcinoma may not be safe.

The Effect of Neoadjuvant Therapy on Early Complications of Esophageal Cancer Surgery
Introduction: Early diagnosis and appropriate treatment is required in esophageal cancer due to its invasive nature. The aim of this study was to evaluate early post-esophagectomy complications in patients with esophageal cancer who received neoadjuvant chemoradiotherapy (NACR). Materials and Methods: This randomized clinical trial was carried out between 2009 and 2011. Patients with lower-th...
متن کاملSurgical Treatment for Esophageal Cancer
Esophagectomy is the main treatment for esophageal cancer. The 2 histologic subtypes of esophageal cancer are squamous cell carcinoma and adenocarcinoma; these subtypes have different biologic features and treatment strategies. Although the prognosis of patients treated with surgery alone remains unsatisfactory, neoadjuvant therapy helps to improve outcome. A meta-analysis revealed that neoadju...
متن کاملRadiation dose does not influence anastomotic complications in patients with esophageal cancer treated with neoadjuvant chemoradiation and transhiatal esophagectomy
BACKGROUND Neoadjuvant chemoradiation might increase anastomotic leakage and stenosis in patients with esophageal cancer treated with neoadjuvant chemoradiation and esophagectomy. The aim of this study was to determine the influence of radiation dose on the incidence of leakage and stenosis. METHODS Fifty-three patients with esophageal cancer received neoadjuvant chemoradiation (23 × 1.8 Gy) ...
متن کاملOptimal surgical approach for esophageal cancer in the era of minimally invasive esophagectomy and neoadjuvant therapy.
The optimal surgical technique for the potentially curative treatment of patients with esophageal cancer is still under debate. The transhiatal esophagectomy (THE) with limited lymphadenectomy mainly focuses on a decrease of postoperative morbidity and mortality by preventing a formal thoracotomy. The transthoracic esophagectomy (TTE) with extended two-field lymphadenectomy attempts to improve ...
متن کاملEvidence-Based Operative Details in Esophageal Cancer Treatment: Surgical Approach, Lymphadenectomy, Anastomosis
BACKGROUND This review depicts surgical treatment strategies in the management of esophageal cancer under the focus of evidence-based medicine. The main emphasis lies on technical details, i.e. surgical approach, lymphadenectomy, and current techniques of anastomosis. METHODS The current literature on operative details in esophageal cancer treatment was reviewed. Surgical approaches and diffe...
